Each character is traced back to the physical key that produced it, then read again from the same physical key in the target layout. A on AZERTY sits where Q sits on QWERTY, so the letters swap in pairs rather than by some alphabet trick. Shifted and right Alt levels are traced the same way, which is why accented letters survive the trip when the target layout has them.
A borrowed laptop set to French while you type English. A remote desktop that forces its own layout. A password field that swallowed your keystrokes under the wrong layout and now refuses you. In all three cases the keys you pressed were right and only the translation was wrong.
A character only converts if the target layout produces it somewhere. The Polish layout builds its nine extra letters with the right Alt key, so they reach AZERTY only where AZERTY has a matching position. Anything without a counterpart is passed through untouched and counted, so you can see exactly how much the conversion could not reach.
